For good and valuable consideration (including without limitation the publicity that I or my child may receive), the receipt and sufficiency of which is hereby acknowledged, I hereby give Rocking Inc., a Delaware corporation (“Rocking Inc.”), its assigns, licensees and legal representatives a worldwide, nonexclusive, sublicensable (through multiple tiers) transferable, fully paid-up, royalty-free, perpetual, irrevocable license and right to use, reproduce, modify, edit, alter, rearrange, prepare derivative works from, distribute, publicly display, publicly perform, digitally perform and publish my name, photograph, image, audio recording, video recording and likeness and, if applicable, the name, photograph, image, audio recording, video recording and likeness of the child named below (collectively, the “Images”) in all forms and manner consistent with applicable law including, without limitation, publication on internet websites, broadcasts and any other publications as released to or by Rocking Inc., including, without limitation, promotional advertising, and other commercial purposes. I understand that Rocking Inc. cannot control unauthorized use of the Images by persons not associated with Rocking Inc. once the Images have been published. I hereby forever waive any right to inspect or approve any publication of the Images by Rocking Inc. I understand and agree that I do not have the right to any compensation or reimbursement from Rocking Inc. in connection with the Images. I expressly acknowledge that Rocking Inc. has no obligation to use the Images.

I hereby unconditionally release Rocking Inc., and its employees, directors, officers, agents, successors, licensees and assigns (the “Released Parties”), from any and all claims, demands, disputes, suits and causes of action, damages, obligations and liabilities, including but not limited to claims for equitable rights and remedies, negligence or gross negligence, defamation, invasion of privacy, misappropriation of publicity rights, and copyright infringement, which I now have, have ever had or may hereafter have against the Released Parties relating in any way to the Images, the processes and procedures used and conduct undertaken in good faith to capture the Images, or the use of the Images.

This Image Release and Waiver is governed by the laws of the State of Florida, without giving effect to its conflicts of laws rules that would result in the application of another law.

I have carefully reviewed and understand the above provisions and agree to be bound by them. I voluntarily and irrevocably give my consent and agree to the terms of this video with submission of video content to Rocking Inc.
